During summer, select the grade the student will be entering

Select your designated school

Select the sport(s) your student is trying out for

Complete Student Information

If you have previously used Athletic Clearance, select the student or parent/guardian from the dropdown menu. Most fields will autofill. Be sure to update any fields that do not.

Complete all required documents.

Be sure to save certificates after each required video.

• Parent permission form (must be notarized)
• EL2 Physical Form-page 4 only (page 5 if necessary)
• EL3 Form (signature completed electronically)
• Impact Concussion Baseline Test or request form
• Proof of Insurance (insurance company name and policy number)

• Heat Illness
• Sudden Cardiac Arrest
• Sportsmanship
• Tackling (Football only-once every 4 years)
• Cheer/Dance Safety (Cheer & Dance only – once every 4 years)
